386 vs 686 kernel? And uninstall linux-image-2.6.15-21-386 removes Linux-386?

linux-386 is a metapackage that (as you say) should always point to the
newest -386 kernel.

Yes

What you could/should do is install a -686 kernel, then remove all your
-386 stuff at once.
