You need:
-Apache (2 is preferred)
-PHP4 or 5
-libapache2-mod-php5 or 4.
mysql files
PHP4 or 5 - mysql/i
After that is all all done and configured(Hopefully you'll get Apache2)
from the command line: sudo apache2ctl restart
Try your file again.
If that still fails. Clear your cache and cookies and try again.
