I just installed a new server, and I found that I had to create everything
(raw partitions, raid-1 array, lvm groups, final formatting) all in the same
installer run in order for lvm on raid to work (no reboots allowed!). I
don't know how much of that is voodoo and how much is necessary, in that I
only did it successfully once, didn't document it, and I was rather stressed
at the time, but it is running now, and the machine boots and has been
running non-stop since then. The only other thing I can think of is that I
stopped and zeroed out the beginnings of both hard drives just before the
last successful installer run, so as to prevent it from detecting the
presence of previous (failed) attempts on the drives.
