The module's name is 'snd-azx'; the filename is 'snd-azx.ko'.

The source code is available in the Debian repository, but I recommend
you wait until the latest (1.0.8-1) is merged into hoary/universe.

Yes. You are able to `apt-get source alsa-source' and create a deb
using the commands in /usr/share/doc/alsa-source/README.Debian.
