I have determined that the problem arises when other programs are
using/have used the sound interface. After rebooting earlier today,
Gizmo worked fine until I had launched other programs which use sound.

For example, if realplay is running or even just open, the
above-mentioned 'Failed to execute child process "audiowrapper"' error
occurs. However, I have now exited realplay and am staring at the
output of ps -e looking for any other program which may be hogging
sound in the current session without luck -- I have also tried "lsof
grep /dev/dsp" without determining the culprit. Any other suggestions
on how to determine which programs are using sound would be most
welcome.

It seems that some Linux programs are better are sharing the sound
interface than others...
