apt- et dist-upgrade

yes, 
did you try to just put in the CD in your running hoary system ??
the update-notifier cares for all the above steps, no need to fiddle
with apt or your sources.list, it will pop up a message that asks you if
you want to upgrade your system if it detects a new ubuntu CD :)
