This is strange.. can you run:
diff -u debian/patches/00list-32 debian/patches/00list-33
and see if some patches have been removed for a mistake.
In theory only one line with your patch should appear.

Did you follow the steps on the how-to one by one (including the dch -i)?

The how-to is missing a bunch of important steps btw that are kinda simplified
in 2.6.10
