either a dieing harddrive or a buggy driver for the controller.

Because you are getting them on a dvd it is far more likely that the 
disk is damaged in some way.

check the disk is exactly how you think it is ;-) you might have a dodgy 
one.  I have had similar errors on removable media in the past and it 
was always a problem with the media ;-)

If you get those errors when no disk is inserted you *may* have found a 
bug somewhere.
