[DC LoCo] IT (equipment) request form

Does any one use an IT requsition form that I can use/modify? Or some ideas that'll help in what I'm trying to do.

My organization spends way too much on equipment, the problem is actually the IT department, we've recently bought duel-core laptops with 4GB RAM, for basic users, who use office and internet applications.

It'd be nice if the form helped managers (over time) to understand user needs, for example if 1 user has 1GB RAM and is happy with the system, the manager is unlikely to authorize 4GB for a new user with similar needs.
