[Ubuntu-US-CA] Placing Team Karmic CD Order

The Team Contact for an approved LoCo makes the request with Canonical.
Individuals can still request CDs, though in much smaller quantities. (Grant
pointed out https://wiki.ubuntu.com/LoCoGettingCds which should have a bit
more info)
