[ubuntu-arizona] Change of Team Channel in IRC

Hi all,

The Community Council has asked all US teams to change the naming of 
their team channels so we all will be set up the same.

Our new channel is: #ubuntu-us-az

The old channel will redirect you if you have auto join enabled on your 
IRC client.
