Your changelog entry contains the following:
  * Merge from debian unstable, remaining changes: (LP: #256980)
  * debian/qwik.init:
    - Included LSB dependency header from Debian #470109.
  * debian/control:
    - Updated Maintainer field as per spec (LP: #230350).
    - Bump standards version to 3.8.0 with changes to the homepage field.
    - Changed suggests libdb4.2-ruby to libdb4.4-ruby

I'd change this to:
  * Merge from debian unstable, remaining changes: (LP: #256980)
    - Changed suggests libdb4.2-ruby to libdb4.4-ruby
  * debian/control:
    - Bump standards version to 3.8.0 with changes to the homepage field.

For these reasons:
 - the change in debian/qwik.init is nothing your revision introduces, also it's not a "remaining change".
 - it's not necessary to mention the changed maintainer field, because we have to do it with every upload anyway
 - moved the changed suggests to remaining changes - it's the only change that's worth keeping.
