[Bug 1033934] Re: Attempting to change regions in the dashboard does not display correctly

This definitely does not affect current Horizon master. In general this
problem is the result of a template failing to extend from the "modal
form" base template, or not using the ModalFormView class. I'm not sure
what's going on in the downstream package to cause this behavior
currently.
