Hi Mark, thanks for taking the time to file a bug report.

Setting up mysql-server-5.1 (5.1.54-1ubuntu4) ...
invoke-rc.d: unknown initscript, /etc/init.d/mysql not found.

If you forcibly removed /etc/init.d/mysql , dpkg will respect that and
leave it gone, as it is considered a configuration file.

Try this:

dpkg --force-confmiss -i /var/cache/apt/archives/mysql-
server-5.1_5.1.54-1ubuntu4_i386.deb

And then

dpkg --configure -a

Marking as Incomplete pending response.

       Status: New => Incomplete
