Looks like this might be a file system consistency issue:
Setting up quota (4.00-3) ...

Please could you try fsck'ing the filesystem first and re-attempt the
install.
Marking 'Low' (I can install quota in precise) and 'Incomplete' pending
outcome of suggested action.
