I've recreated this with a ttylinux based image, with modifications to access metadata service early in boot.
I'm attaching that instance here.

this can be registered with 'uec-register-tarball ttylinux-11.0-fastboot-amd64-0.07.tar.gz mybucket'
it will also boot in smaller than m1.small (ie, you can shrink m1.small to 1G fs and 96 M mem)
