Do this test, exec this command:
slapd -d 1
and search in the last lines if you have this error:

bdb(dc=YOUR,dc=DOMAIN,dc=net): mmap: Invalid argument

Because i had the same error of you... and i found that my kernel is not compiled with mmaping (had the same error on "standard" rrdtool from repositories)
If you have this error, you can do 2 things:
- compile openldap from source, disabling mmap
- install an mmap enabled kernel
