Review: Needs Information
Trying to ascertain the history of apache2-mpm-prefork definitively is a bit challenging. I gather it was a php5-era source package that is long since gone? rmadison has no mention of it. I can at least confirm it no longer appears in the groovy archive:
$ apt-cache policy apach2-mpm-prefork
N: Unable to locate package apach2-mpm-prefork
The php7.4 debian package still has some vague references to prefork but doesn't appear to have direct dependencies. I wasn't able to find direct mention of it in debian/changelogs though. It sounds as if maybe prefork got rolled into apache itself?
samba-doc looks like it exists for precise and trusty, but rmadison and apt make no mention of it. Changelog indicates it was removed in samba (2:4.3.3+dfsg-1):
* Remove the samba-doc package, and move examples files from it to
relevant other packages. Closes: #769385
The rename of btrfs-tools to btrfs-progs occurred at 4.4.1-1.1 in 2016, per its changelog:

Regarding the server-ship drop, that really feels like it should be a separate MP, especially given the dependence it has on MP #389947. I think if you dropped that change from this MP, you could land this MP immediately (apart from my question on the prefork module everything looks good to me.)
I'd also prefer to see some analysis that dropping server-ship doesn't cause anything to get lost. I see from MP #389947 that there has been some thought given to that, and looks like this and other MPs are addressing the discrepancies that have come to light, so much of that work is done, it just needs a happy summary that everything's accounted for, and nothing will break with server-ship's removal. (I know at least this will break my merges board... but that's my own problem to fix, and I should take the opportunity to make its seeds query code more robust...)
