Changes:  LP: 1422113 - fixed missing icons in Ubuntu GHOME by updating both information and warning legacy icon references

Rationale:  Last dialog shown during ubiquity install had missing "information" dialog box icon because of outdated icon reference.  "Warning" dialog box icon references were also outdated.

Test:  Build from source on vivid (I used sbuild vivid environment on trusty).  Run LiveCD for Ubuntu GNOME vivid.  Select "Try Ubuntu" then copy all the .deb files from the build over to a subdirectory on the LiveCD file system.  Install all .debs using e.g., "sudo dpkg -i *.deb" or the equivalent.  Dependencies failed using dpkg so I went ahead and had to subsequently do "sudo apt-get -f install" to fix.  Then, run ubiquity to completion and you will see the "light bulb" dialog-information icon instead of a placeholer for a missing icon.
