I am preparing this memorial plaque to take with me to Poland, but I 
have lost contact with my people there and I need a definitive text 
within the next 6 hours so the plaque get made before I travel!

could you please check the text below and maybe correct it? The 
important part is the last 5 lines. I have a Google translation, but do 
not know enough Polish to be sure it is accurate.
