[ubuntu-mythtv] proper way to generate a patched kernel

I'm working on a appletv install of mythbuntu.  The appletv requires 
several kernel patches in order to boot. These patches are specific 
to appletv hardware and will never be accepted into the mainline 
kernel so I need to create a special kernel for appletv usage.

What is the proper way to create a patched kernel that can be used to 
replace the standard kernel installed under mythbuntu. Basically a 
replacement kernel that still works with the standard mythbuntu
