Update - Resolved
As of today (or yesterday - not absolutely sure), this problem I
believe has been fixed (by recent updates?).
In scenario #2, if I close out Firefox and then quickly reopen, the
message box re "Firefox still running" will appear; however, if I close
that message box, a "spinner" progress indicator will appear. Now, if I
wait until the spinner disappears, and then reopen Firefox, it opens
Likewise, for scenario #1, if I attempt a shutdown of Ubuntu too
quickly after closing Firefox, I will get the message "A program is
still running: ..". If I close this message, a spinner progress
indicator as above appears, and I can successfully execute a shutdown
after the indicator disappears.
Evidently, it is necessary to allow Firefox sufficient time to unwind
the websites (tabs) that are displayed in the window or these messages
will appear. The fix is that it is not necessary to restart Ubuntu to
reset Firefox - just wait until the spinner progress indicator

You have (or maybe just typed) The directory where both lock files are located wrong. Copy/paste these commands:
cd `ls | grep default`
Now you should be in the right directory.
