Public bug reported:

Binary package hint: flashplugin-nonfree

Load a flash applet (go to YouTube, for example).
Change the focus (switch windows).
Change back (go back to the window with the YouTube page open).
Flash has stopped working and the page needs to be reloaded.

Also seen this happen when trying to install BBC iPlayer Desktop, which
means I have to sit and watch it install and cannot do anything else
while I wait.

This is a major PITA.
