Please understand that "exclusively" in the phrase you quote means that
the character set should logically not be described anywhere else than
within the HTML file. Hence, what is important with regard to this
sentence is not what is inside the file but what is outside of it, or
rather that nothing is there, or rather that it is not used.
Your concern about a character set tag existing or not inside the HTML
file pertains to the earlier discussion of the fall-back character set.
