Public bug reported:

Binary package hint: mozilla-firefox

I was using firefox and then a blank window (it could be resized, and it
had normal decorations including the top-left icon, minimise, maximise
and close buttons) appeared. I wasn't quite sure what program it
belonged to so I ran xwininfo on it:

xwininfo: Window id: 0x3800001 (has no name)

  Absolute upper-left X: 174
  Absolute upper-left Y: 484
  Relative upper-left X: 5
  Relative upper-left Y: 24
  Width: 300
  Height: 250
  Depth: 24
  Visual Class: TrueColor
  Border width: 0
  Class: InputOutput
  Colormap: 0x20 (installed)
  Bit Gravity State: NorthWestGravity
  Window Gravity State: NorthWestGravity
  Backing Store State: NotUseful
  Save Under State: no
  Map State: IsViewable
  Override Redirect State: no
  Corners: +174+484 -1126+484 -1126-466 +174-466
  -geometry 300x250+174+484

And then xprop:

_NET_WM_ICON_GEOMETRY(CARDINAL) = 222, 1176, 188, 24
_NET_WM_STATE(ATOM) =
WM_STATE(WM_STATE):
                window state: Normal

>From that, I couldn't tell that it was Firefox, so I started closing programs, and when I closed firefox, the window closed. Sorry I can't be more specific or know how to reproduce it. I have the following extensions enabled:

DownloadHelper 3.0.0
DownThemAll! 1.0.1
Flashblock 1.5.5
Tab Clicking Options 0.6.9
Ubuntu Firefox Modifications 0.5

My plugins are:
Default Plugin, Demo Print Plugin for unix/linux, DivX(r) Web Player, Helix DNA Plugin: RealPlayer G2 Plug-In Compatible (compatible; Totem), iTunes Application Detector, Java(TM) Plug-in 1.6.9_06-b02, QuickTime Plug-in 7.2.0, Shockwave Flash, Totem Web Browser Plugin 2.22.1, VLC Multimedia Plugin (compatible Totem 2.22.1), Windows Media Player Plug-in 10 (compatible; Totem)

This is Firefox 3.0b5 (3.0~b5+nobinonly-0ubuntu3). Ubuntu 8.04

If it happens again, I'll try to work out a way to reproduce it.
