This process worked for me to duplicate the bug every time:
1) start w. a fresh profile (which will have Ubufox installed by default)
2) goto AMO and install something with a toolbar icon... I chose Update Notifier since it's one of the things I run
3) restart Firefox and drag the toolbar icon over by the throbber
4) go back to AMO and get TMP

If you get the same results I do, when you restart Firefox, the Update
Notifier icon will be gone, the throbber will be gone, and the icon
library in the "customize toolbar window" (whatever that thing is
officially called) will NOT contain those missing icons.

And if you disable Ubufox, the icons come back.

It's not an issue of corruption in localstore.rdf. It's almost like it's
being entirely ignored.

If it's in any way relevant, I'm on an amd64 system, running Intrepid
and Gnome.


(I don't have time at the moment to play with screenshots, but like Myk Dowling said, it's just going to show the standard layout.)
