[Bug 112814] Stock GNOME button icons missing

Public bug reported:

Binary package hint: firefox

Since the most recent Firefox uprade (approximately), I no longer get
stock icons on dialog buttons.  As one of many examples, the HTTP
authentication dialog doesn't have the standard OK and Cancel icons.
