Yes, and the package should be versioned with -0ubuntu1. For example if
the Debian package in NEW is 4.5.6-1, the Ubuntu upload would need to be
4.5.6-0ubuntu1.
I don't recall any other requirement, though I do find it hard to
remember everything without the package in front of me to prompt me.
Storing the changes in git is not required, but many do - for example
it's quite common to have Ubuntu packaging branches in Salsa.

IIRC, Debian doesn't publish its NEW queue - possibly because it hasn't
passed copyright/licensing review. You'll need to supply it for
sponsorship some other way. Ubuntu sponsors generally don't mind how it
is done, as long as they can easily grab the proposed source package for
review and upload. You could use a PPA for this for example, or some
file or VCS hosting somewhere else.
