Je pense que le probleme reside a ce niveau :

-->The PPP daemon has died: A modem hung up the phone (exit code = 16)
--> man pppd explains pppd error codes in more detail.

regard ce thread je pense que ca resout le probleme ==>

sinon pense a changer le port usb :)
