Could you add DEBCONF_DEBUG=5 to the kernel command line and get me the
resulting /var/log/syslog? You'll probably want to send it privately
and/or use a dummy password, as it'll log *everything* done via the
debconf protocol.

If you're fetching the Kickstart file over the network, the installer
has to bring up the network earlier than usual, which is the source of
the first time. I don't remember whether this resulted in two DHCP runs
when I originally wrote this, though ...
