I actually took the opportunity to write a self-contained perl script that
makes it trivial to solicit user comment. installation time = 1 minute.
 It's not strong and feature-full enough for ubuntu, but it does most of
the job for me.

maybe someone wants to pick it up and make something better out of it.  it
is in action at http://book.ivo-welch.info/ed2/ ,  click "Leave Feedback"
at the bottom.  The source of the perl script itself can be viewed at the
bottom of the comment leaving form.
