Adding footnotes

With dblatex, the header and footer are described
in /usr/share/texmf/tex/latex/dblatex/style/dbk_headfoot.sty but I'm not
familiar enough with LaTeX to insert/scale the image. I know the line is
\includegraphics[options]{filename} though.
