When someone commits a patch to the DocTeam SVN repository [1], a
notification email is sent to the ubuntu-doc-commits list, which the
ubuntu-doc list is also subscribed to. This email contains the entire
patch, along with some other details about the commit.

The patch itself is in the form of a 'diff' of DocBook XML, which is
only slightly human readable. You can find out more about diff on
Wikipedia [2].
