[PATCH] Entities and validation

The goal here was to track down literal references to Dapper's version and 
replace them with entities.  This led to discoveries of some other problems, 
like global.ent not being sourced in some files, kubuntu-specific entities 
(like kdistro-rev) that wanted to be replaced with global ones, invalid 
entity names (like wiki-RootSudo), and some other validation problems, all 
pretty much fixed with this series of patches.  I learned a thing or three 
about docbook and XML while I did this, so I'm going to mass-validate the XML 
and fix whatever else I can after this.

I think I'm going to have to spread this across multiple emails due to size 
limitations on the list...
