Changes to kubuntu-docs

As per discussions off list with Jonathan Riddell, I've committed some
changes to the kubuntu section which make the builds use xsltproc for
building the html for the distribution. This means that the same build
system is now used for the kubuntu docs which are in the distro, as
those which are on the website. 

The Makefile has been simplified considerably, because there is now just
1 entry for each document, rather than three. Also, kubuntu docs can now
use xincludes rather than entities for adding extra files, which I would
highly recommend changing to for translation purposes.
