Revamping the help system

Hi,

I've added index-mdke.xml to the repo which is a sort of middle ground -
it has a short list of sections with some examples of what is inside
listed underneath. The full content of the sections is then in a
subsection which can be viewed by clicking the relevant section title.
It also has the advantage that the sections appear in the yelp side bar.

Obviously at the moment it has a double index because the xsl that yelp
uses generates an automatic table of contents at the top. However
upstream has said that it would be quite easy to permit individual
documents to repress that table of contents, so we could remove that if
this sort of format is the way we want to go.
