A couple of DocBook style questions

Another style I think we should adopt: as discussed on irc earlier today
with Brian: the method of advising users to install packages is dodgy at
the moment. Instead of:
[quote]
* Add the X repository
* Install:
Programming (universe) -> Bluefish
[/quote]
We'd like to do this:
[quote]
* Install <application>Bluefish</application> from the Multiverse
repository (see <xref linkend="add-applications"/>).
[/quote]
I'd also suggest that we use the <filename> tags around names of
repositories, because that makes them stand out a bit, in monospace,
which is nice I think.
As usual, voice any fierce objections here.
