Updated makefile for the book

Hello,

the previous invocation of xltproc was a bit imprecise: I have updated
the makefile so that now:
 - it generates the html output files in a subdirectory
 - invoking make twice doesn't rebuild, unless something changed

This has been done during the debianization effort for the book.  I hope
I didn't cause inconvenience by moving the place where the output gets
created.
