Switching to AMD64

I'm currently running a k7 kernel on my k8.  I've seen a lot of
discussion of AMD64, but I can't find the single place to look for
details of:

- what commands to run to upgrade from i686 to AMD64 in a working hoary
  install
- what might break if I do so

Can anyone point me to some links?
