For some reason it's only possible to get the Push-to-Select to emulate one
of the other three buttons (although I was investigating wether you could
get the data separately by switching it into debug mode and dumping the
registers).

I appreciate the 3 buttons;  but I know some people appreciate scrolling, so
you can do: 

  Option "Emulate3Buttons" "true"
  Option "EmulateWheel" "true"
  Option "EmulateWheelButton" "2" 

Which leaves you with:

  Left             = Left
  Left + Right     = Middle
  Right            = Right
  Middle + Nipple  = Scroll
