https://wiki.ubuntu.com/StableReleaseUpdates. You should file a bug
describing the problem you're fixing to start with (if there isn't
already a bug for this issue).

For updating a stable release, see the procedure documented in the
second link.

When you have packaging updates ready, please attach them in the form of
debdiffs to the bug and subscribe ~ubuntu-sponsors - though for updating
Xenial, make sure you follow the SRU procedure (documented in the second
link), otherwise your upload request will be rejected.
