What does it mean to clear out and old kernel version? When I upgrade a kernel, I either available kernels from apt or download and install a .deb kernel package.

Clearing out old kernel versions manually to be able to upgrade the kernel version is something the end user should never have to do. Clearing out old kernel version from /boot should be better managed by the software updater to intelligently manage historic kernel versions based on available disk space on the partition.
