Minor change to freeze exception procedures

Up until now, freeze exceptions have been mailed to the release
manager, with a Cc to the backup release manager.  This has caused
some delays and other problems so I decided to tweak the procedure.

All freeze exception requests should now be filed as bugs against the
relevant package in launchpad and the ubuntu-release team should be
subscribed to the bug.  The requirements as to which information
should be included are the same.

If you feel that a request is particularly urgent or you want input
from a release team member, do feel free to contact either me or
another team member on IRC or via email.
