Ubuntu Kernel Team bug triage policies

This policy is meant to work in conjunction with current bug triaging
policies. This is in the hopes that the bug flow to the kernel team will
be easier, and allow community to better help the team with the large
amount of bug reports we receive.

I suspect I'll be making more adjustments to this document, but it's a
fair first run.
