[ubuntu-cloud] Trouble with multiple instance deployment

I can successfully deploy a small instance but when I attempt to deploy a
second it never seems to become available.
It is listed as "pending" and with the IP address 0.0.0.0
When I show availability zones it shows I have the capacity to run 6 small
instances, with 5 remaining.
By looking at logs I can see that all nodes are connecting.

My setup is as follows:

ASA issuing DHCP in the range 192.168.1.200-254

PC1: Cloud controller/cluster controller/storage controller
Single NIC: static IP below dhcp range
Elastic IP range for instances configured as 192.168.1.10-20

Node1 (dual core/vt enabled): Role: node controller network: dhcp
Node2 (dual core/vt enabled): Role: node controller network: dhcp
Node3 (dual core/vt enabled): Role: node controller network: dhcp
