Remove the usb thumb drive.
Go to synaptic package manager and reinstall, udisks & usbutils.
Then edit, fix broken packages in synaptic.
Update
Try again to format.
If it still does not work open terminal while your usb is plugged in, type:
"sudo lsusb" (no quotes) while your usb is plugged in.
Enter your password, enter.
lsusb sometimes wakes it up.
