USB mouse continually disconnects and reconnects

That it has happened on multiple OS versions makes me think it's a
hardware issue with the mouse, or maybe with your USB ports.

Have you tried connecting it to a different computer and seeing if the
same thing happens?  Apologies in advance if you've already tried this
