OPENOFFICE BASE

Can anyone give me some pointers on how to attach a data source to a 
form, either in a reply to this post or by reference to a valid 
posting?  The form wizard does not work.  Apparently that's an upstream 
Debian bug.  But I cannot find any current instructions on how to attach 
a data source to a form manually.  There are all kinds of articles 
available, but none seem to match the realities of what's on my screen!

Help!
