You must NOT use cdrwtool to format DVD! It's only for CD-RW media (as the name implies).
The procedure is this:
1) If you haven't already used a DVD media (it's virgin) then you must format with: sudo dvd+rw-format /dev/$YOURDVDBURNER
2) Put a udf filesystem into your media with:
sudo mkudffs --spartable=2 --media-type=cdrw /dev/$YOURDVDBURNER
