[Bug 898309] [NEW] bluetooth adapter not properly initialized

Public bug reported:

I clicked the app-indicator for my bluetooth adapter and It only have me
the option to turn it off, and open the preferences.

If I open preferences, it tells me that bluetooth is disabled. the upper
left slider is in the off position but I can't turn it on.

Also:
$ hcitool dev
Devices:
	hci0 	00:00:00:00:00:00


This happens regularly, usually after a reboot and sudo service bluetooth restart  does nothing useful.

dmesg shows:
